If you would like to have a plaque in our garden to celebrate someone you love, honour the memory of a loved one, or celebrate birthdays, weddings and special events, please complete the details below and we will create your plaque for you.
 
Your plaque will be added to our special wall that you can visit at any time. Your plaque will be displayed for 2 years, at which time Sage House will contact you and you can choose to renew your gift for a further 2 years or we can send your plaque to you so you can treasure it at home. Through the purchase of your plaque, you will be making a lasting difference to families living with dementia.
 
Plaque size approx: 155mm x 120mm (6”x 4½”)
 
You can order more than 1 plaque if you would like to, providing we have the space to fulfill them. There is naturally however, limited space available. Or, you can order additional ones for your home.
